We are seeking a primary care provider to see participants in a variety of community-based settings including clinic, assisted living, memory care, and private residences. Board Certified/Board Eligible in Family or Internal Medicine
Able to obtain Oregon Medical License (DO or MD)
BLS for Healthcare Providers from American Heart Association
Able to obtain DEA license
Clinical experience working with medically frail, elderly population
Certification in Geriatrics and/or Hospice & Palliative Care
